Micronized Fat Treatment (MFAT): A Gentler Alternative for Joint Pain

Micronized Fat Treatment (MFAT): A Gentler Alternative for Joint Pain

Among the newer regenerative options for joint pain, Micronized Fat (MFAT) treatment has drawn growing interest — both from patients looking for alternatives to surgery and from clinicians comparing it to older cell-based therapies. Here’s what it actually involves.

What Is MFAT?

MFAT uses a small amount of your own fat tissue, usually taken from the abdomen or thigh through a minimally invasive liposuction technique. Unlike older fat-grafting methods, the tissue is mechanically broken down — “micronized” — into much smaller particles without the use of enzymes. This processed fat is then injected into the affected joint.

The appeal of this approach is that adipose (fat) tissue is naturally rich in structural and signalling components that may support the joint environment, and because the procedure uses your own tissue, it avoids the risks associated with donor or synthetic materials.

How Is It Different from Stem Cell Therapy or SVF?

You may have heard MFAT discussed alongside terms like “stem cell therapy” or “SVF” (Stromal Vascular Fraction). While related, they aren’t identical:

  • SVF typically involves enzymatic processing to isolate a specific cellular fraction from fat tissue.
  • MFAT uses mechanical (non-enzymatic) processing, keeping more of the native fat tissue architecture intact.

The two approaches are being actively compared in clinical research, and which is preferable can depend on the specific joint, condition, and clinical goals — this is an evolving area, and treatment recommendations should be individualised rather than generalised from marketing claims.

Who Might Be a Candidate?

MFAT is generally considered for patients with joint pain — most commonly knee osteoarthritis — who want a non-surgical option and have enough donor fat tissue available for the procedure. As with any orthobiologic treatment, candidacy depends on an accurate diagnosis and staging of the underlying joint condition, not just a general desire to “try something regenerative.”

What to Expect

The procedure is typically done in a single outpatient visit: fat is harvested, processed, and re-injected into the joint under ultrasound guidance, usually within the same session. Recovery is generally quicker than fat-grafting techniques of the past, though your clinician will guide you on activity restrictions specific to your case.

The Bottom Line

MFAT is one of several tools in the regenerative toolkit — not a universal fix. Its value depends on matching it to the right patient and the right stage of joint disease, which is why a thorough evaluation should always come before treatment, not after.
